Tiens, en parcourant ça, je tombe sur le rapport de Feynman sur son investigation à la NASA après l'explosion de Challenger.
Il n'avait pas l'habitude de faire dans la langue de bois, mais là, putain comment ça casse. :D

Après ce rapport, ça a saigné, il y a eu un sacré paquet de têtes qui ont roulé dans cette administration, et la NASA l'a mise en veilleuse pendant plusieurs années (grosse réduction de budget inside). Mais je suis sûr que le phénomène décrit (érosion des procédures de contrôle due à une confiance exagérée en l'équipement sur la seule base de "ça a marché correctement jusque-là" ) s'applique à quantité d'accidents majeurs, comme Tchernobyl.
Truc amusant, il apparait que le système donné en exempleà suivre d'un point de vue qualité est le système informatique, bien qu'apparaissant complètement obsolète.
